We are open:
Monday – Thursday from 9:00 am –  6:00 pm (closed for lunch 1:00 – 2:00pm)

If you need to be seen during our non-operating hours, please go to a local Urgent Care or Emergency Department. If your concerns can wait until we are open, we would love to help you then.

Same day appointments are available. To make an appointment you can call the office at 385-645-7474.

Most Insurances are accepted, including Medicare, Medicaid, and cash discounts for self-pay at the time of service.  (Medicare AARP & Humana are not accepted at this time)
